Knowing which form to use to access your military benefits can be confusing.

CSU Global’s expert military-specific staff can help you decipher what is required for each benefit type. Review the documentation requirements below, and if you have any questions, please contact your student success counselor.

If you are unsure what type of benefits you should receive, you can get that information from the U.S. Department of Veterans Affairs. You can also access the VA’s online application (VONAPP) through the VA benefits website. If you need a different form, search the VA’s form database .

  3. Verification of eligibility

Chapter 30 Montgomery GI Bill Active Duty (MGIB)

  • DD-214*
  • Application for Benefits (VA Form 22-1990)*
  • Certificate of Eligibility (VA Form 22-1993a) (upon receipt) (in lieu of 22-1990 if available)
  • Change of Program or Place of Training (VA Form 22-1995) (If benefits were previously used elsewhere)

Chapter 31 Vocational Rehabilitation

  • DD-214*
  • Authorization into Rehabilitation and Certification of Status (VA Form 28-1905)*

Chapter 33 Post 9/11 GI Bill

  • DD-214*
  • Application for Benefits (VA Form 22-1990)*
  • Transfer of Eligibility (VA Form 22-1990e) and DOD Approval (If applicable)
  • Certificate of Eligibility (VA Form 22-1993a) (upon receipt) (in lieu of 22-1990 if available)
  • Change of Program or Place of Training (VA Form 22-1995) (If benefits were previously used elsewhere)

Chapter 35 Dependents Educational Assistance (DEA)

  • Application for Benefits (VA Form 22-5490)*
  • Certificate of Eligibility (VA Form 22-1993a) (upon receipt) (in lieu of 22-5490 if available)
  • Change of Program or Place of Training (VA Form 22-5495) (If benefits were previously used elsewhere)

Chapter 1606 (MGIB-Selected Reserve) & 1607 (REAP)

  • Application for Benefits (VA Form 22-1990)*
  • NOBE (Notice of Basic Eligibility) DD Form 2384*
  • Change of Program or Place of Training (VA Form 22-1995) (If previously used benefits elsewhere)
